I want to point that any request form that is incomplete will be rejected. I already received some blank request forms. You MUST tell me what you are requesting, I’m not a mind reader. I can not send you anything if you select nothing. It is multiple choice, you can select more than one item. There will be some delays on filling request orders. I am very busy this week and will be busy early next week. When I finally have the time, I will send downloads in whom who completed the form as soon as I can.
